Hematomas after performance of the face lifting: pathogenesis, classification, prophylaxis


Abstract

Objective. To study the pathogenesis and to create classification of the subflap hematomas, to compare the rate of their creation after performance  of tense paramedical facial lifting and typical SMAS-plasty, to elaborate prophylactic measures.

Маterials and methods. In the investigation 300 patients took part, in whom face lifting was performed. The control group consisted of 150 patients, to whom the tense paramedial SMAS-lifting was done, and the investigation group - 150 patients, in whom typical SMAS-plasty was conducted.

Results. In a control group the generalized hematomas have occurred in 12 (8%) patients. In the investigation group the generalized hematomas were not observed.

In a control group the localized hematomas have occurred in 30 (20%) patients, of them in 12 - in region of lateral buccal surface, in 13 - in retroaurikular, in 5 - in temporal space. In the investigated group the localized hematomas have occurred in 9 (6%) patients in retroaurikular zone.

Conclusion. As a result of the investigation conducted there was established, that the procedure of tense paramedical SMAS-lifting together with sufficient possibilities for repositioning of the cover structures of the face, significantly minimizes a free blood accumulation under the tissue flaps and excludes the development of serious complication - generalized hematomas.
