PECULIARITIES OF DIAGNOSIS, PREOPERATIVE PREPARATION OF SURGICAL TACTICS IN PATIENTS IN THE OBTURATION JAUNDICE SYNDROME
The results of surgical treatment in 272 patients, suffering diseases of pancreatoduodenal zone, complicated by hyperbilirubinemia, were analyzed. Pancreatic head cancer was diagnosed in 174 (63.9%) patients, cancer of duodenal papilla magna - in 20 (7.4%), cancer of distal common biliary duct - in 24 (8.8%), chronic pseudotumoral pancreatitis - in 54 (19.9%). Теrminolateral anastomosis in accordance to Whipple was constructed in 38 (13.8%) patients, terminolateral anastomosis in accordance to Shalimov-Kopchak - in 40 (14.6%), pancreatojejunoanastomosis with construction of ductomucosal anastomosis - in 128 (47%), pancreatogastroanastomosis with invagination of pancreatic stump into gastric remnant - in 35 (12.8%), pancreatogastroanastomosis with immersion of pancreatic stump into sleeve, constructed from gastric big curvature - in 32 (11.8%). The pancreatodigestive sutures insufficiency have had occur in 32 (11.7%) patients. Lethality was 5.1%.
